Massage Forms for People

New Massage Clients please fill out and sign these four forms:

Confidential Client Information and Health History, Financial Policy, Informed Consent for Massage, and Privacy Policy

If you are pregnant please fill out the Pregnancy Questionnaire.

If you have cancer now or had cancer within the last two years please fill out the Cancer Questionnaire.

If you need me to release you records to another party (i.e. insurance company, chiropractor, doctor, etc.) please fill out the Records Release Form.

I love getting feedback and would appreciate any that you may have to give.  If you care to provide me with any constructive criticisms or suggestions please fill out the Client Response Form.

Massage Form for Animals

Please fill out an Animal Information and Health History Form for each animal receiving massage.

To  participate in the yoga lessons (currently private lessons only) please fill out the Student Consent Form.
